Together with the person served and their team, the Program Manager develops long and short term goals, incentives and programs to help the individual achieve greater independence and live their life, their way!

Job Duties May Include: • Encouraging and respecting personal choice and decision making • Manages authorized initiation, scheduling, and delivery of all contracted services 1. Coordinates the delivery of all services according to the rights of persons receiving Home Care Services 2. Coordinates staff scheduling for delivery of all authorized methods to achieve service outcomes • Maintains and performs periodic audits of consumer case files in order to ensure accurate documentation of program related delivery of services. • Processes incident reports including accident reports and reports on the emergency use of controlled procedures. • Makes recommendations about the type and amount of Home Based Services and in consultation with the guardians, coordinates the start up, delivery and continuing schedule of all home based services including habilitation, respite, physical adaptations and consulting services. • Serves as the assigned QMRP and performs all approved control procedure coordination, intervention, monitoring, evaluation, and reporting functions. • Oversees the training and supervision of Direct Service Professionals (DSP’s). • Administers and coordinates medical or health related services • Monitors physical site including, maintenance, cleaning, utilities, adequate furniture and furnishings, and neighborhood relations. • Administers the financial resources of the consumers program and petty cash accounts. • Coordinates the expense control, utilization and program related billing of all services. • Coordinates the effective completion of time records, billing sheets and produces timely and accurate billings of all collateral and direct services. • Is directly responsible for the accurate documentation of all billings. • Coordinates the prudent spending and documentation of assigned financial resources within the limits set by the company. • May perform job duties as described in the DSP and DSP2 job description. • Responsible for other job duties as assigned
Position Requirements: • Bachelor’s Degree and one year of experience working with individuals with disabilities or related conditions sufficient to obtain a rating of Qualified Mental Retardation Professional (QMRP) • Proven ability to coordinate the delivery of habilitation services to adults with developmental disabilities • Proven ability to develop measurable, data based habilitation plans • Proven ability to supervise others in a human services related field.
