CCP Family Services provides Contracted Case Management throughout
Ramsey, Hennepin and Scott Counties to individuals on one of the following County Waiver
Programs: Elderly Waiver (EW), Community Alternatives for Disabled
Individuals (CADI), Traumatic Brain Injury Waiver (TBI), MR/RC or the Alternative
Care Program(AC).
Case Management Services include, but are not limited to on-going assessment of individual needs, care plan development, resource referral, service coordination, and advocacy on behalf of the individual. The purpose of Case Management is to provide service coordination for vulnerable adults and children in hard-to-navigate service delivery systems.
CCP Family Services has Licensed Social Workers, fluent in Spanish, Hmong and English to meet your Case Management needs.
