CCP Consumer Directed Services has certified Flexible Case Managers and staff, bi-lingual in Hmong, Spanish and Somali, ready to meet your needs. Flexible Case Management can be purchased using the individual's waiver budget. A Flexible Case Manager can assist with such things as:
- Provide information about CDCS and provider options
- Develop, implement, monitor and revise the support plan
- Assist in recruiting, screening, hiring, training, scheduling, monitoring, and paying workers
- Monitor the services including such things as interviews or monitoring visits with the consumer or service providers
- Provide staff training that is specific to the consumer’s plan of care
- Provide ongoing support and problem solving
