Welcome to CCP Self Directed Services

CCP Self Directed Services is a Fiscal Support Entity (FSE) that acts as a link between your services and the funding. Under the Consumer Directed Community Supports (CDCS) option, you will create a plan to meet your needs. We will assist by reimbursing you for items approved on your plan, act as the employer for your staff or help you become an employer, and helping you manage and monitor your budget.

CCP began providing services to people with disabilities over 30 years ago. Since that time, we have continued to evolve our services to find creative ways of meeting the individual needs of the people we support. This approach has led to the development of innovative housing models, supported employment services, customized one to one supports, as well as our self directed programs. Our philosophy of service is based on the idea that the individual knows best what they want and need, our role is to help make that happen.

Funding Sources: CCP Fiscal Support Entity (FSE) services are typically paid for through Consumer Directed Community Supports (CDCS), Personal Supports, or the Consumer Support Grant. CCP also works with insurance, health plans and accepts private pay.

CCP offers you three FSE options to help you manage your waiver funding. In choosing a service option, you decide how much responsibility you want to take on and how much you want CCP to do.
